*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
At Grasslands Dental Care, making sure you understand the state of your oral health is our number one priority. The more you understand the better able you'll be to make informed decisions about your ongoing oral health.
That's why we often recommend a separate, private consultation where we can review x-rays, oral health scan and other findings as well as walk you through your options if more than one method of treatment is required. Of course, an estimate of the fee for any service will also be given at this appointment.
We strive to have the best communication possible with our patients. We will do our best to keep you fully informed about your oral health, your dental treatment options, and your financial responsibilities. If you ever have any questions feel free to ask Dr. Oppenheim or any of our staff members.
At Grasslands Dental Care we make it our policy to prepare a clear and simple estimate of the costs associated with any treatment we recommend. This way, you know exactly what to expect and how much it will cost. No surprises.
We require payment at the end of your appointment. While we accept most dental plans, it's important to check that we accept yours prior to your appointment. Also, some dental plans have been reducing the amount of coverage they provide so it's always a good idea to check in with them and make sure your services are covered. Grasslands Dental Care is not responsible for any charges not covered by your dental plan.
